It's also possible they developed a tolerance and do not realize it. Maybe they went through a rough time and took more than usual for a while and now they're expecting the old dose to work.
Also, there's nothing wrong with taking ibuprofen at the same time as acetaminophen. Just tell them to take an Advil with it next time.

I actually CWE as /u/drunkraccoon suggested but she's adamant that the only thing that works is Vicoprofen, lol. Definitely in her head.
Her doc just switched her to Dilaudid. I'm going to relay this information to her! My advice to her re: Dilaudid (from what I understand it's literally hydromorphone HCL with no cuts) was to just take ibuprofen with it.
I don't think I've ever seen hydrocodone without acetaminophen or ibuprofen in a pre-made pill. Oxy, yes, but not hydro. Not saying it doesn't exist but I've never heard of it.
The person should just go to a compounding pharmacy. But they're doing a good job at getting the doc to prescribe better drugs so I'd keep rolling with it not working if I was them.
Unfortunately her insurance provider doesn't cover compounding, but their "pain management" is pretty wild with what they're willing to prescribe. I myself am personally interested in the oxymorphone which you mentioned above.
As far as the massive 60ct 7.5mg norco's, gonna definitely CWE at least half if not all of those to volumetric dose. Definitely want to try railing a line of the Dilaudid too. What's your personal ROA?

But her doctor says that is not on the "formulary" so that can't be prescribed. But I'm confused between the difference of Zohydro and Dilaudid as both seem to be /just/ hydromorphone?
