For the phone call to the carrier, I would use textnow on tor and call them through that. Use a burner outlook email address to sign up. Try to use some form of a voice changer if you expect to do this method often. Try not to use the same carrier too often, switch each time.
3. For cashout, buy a crypto voucher on your setup. When the bank sends a otp, you will get the code because you simswapped. From there, just visit the redemption page of the voucher provider, enter the voucher code and your burner email address, and select btc and redeem. From there, put the btc through a mixer, then convert to monero, then convert back to btc and send in small increments to your personal wallet and sell for fiat, or just use a monero or btc gift card with cake wallet on things you want. Either one works.
